Pathways Healthcare is seeking a Registered Nurse for home health coverage in Marlborough and surrounding towns. The role runs Mon–Fri, 8:30–5:00, with a strong training program for new nurses. You’ll deliver patient-centered care and coordinate with interdisciplinary teams.

Qualifications include 2+ years nursing experience (geriatrics preferred), BSN, EMR proficiency, BLS CPR, and a valid driver’s license. A comprehensive benefits package and professional development opportunities are offered.

Qualifications

• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Current BLS CPR certification.
• Valid driver's license and own insured vehicle.

Responsibilities

• Assess and evaluate patient status; write and initiate plan of care.
• Re-evaluate patient and family needs; revise plan of care as needed.
• Develop and implement goals-based care plan with nursing diagnosis.
• Administer medications and treatments per physician’s orders.
• Maintain accurate clinical notes and track pain/symptom changes.
• Communicate patient needs and changes with physician."

Job Description:

• Role: Registered Nurse - Home Health
• Service Area: Marlborough, Hudson, Stow, Maynard, Bolton and Sudbury areas.
• Schedule: Monday to Friday (8:30-5:00pm)
• Job Type: Full Time

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• New nurses, feel free to apply! We have a great training program.

Responsibilities:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
