Job summary

Pathways Healthcare is a physician- and nurse practitioner–led home health and hospice organization delivering patient-centered care at home. We integrate AI-assisted documentation to save clinicians time while keeping clinical judgment front and center.

Qualifications

  • Degree in Nursing from an accredited institution.
  • 2+ years nursing experience; geriatrics preferred.
  • Proficient with electronic medical records.
  • Excellent bedside manner and communication skills.
  • Working knowledge of health care laws and regulations.
  • Maintain current BLS CPR certification.
  • Must possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

  • Assess and evaluate patient status; write and initiate plan of care.
  • Regularly re-evaluate patient and family/caregiver needs.
  • Revise care plans as necessary; involve patient and family.
  • Develop care plans based on nursing diagnosis and goals.
  • Administer medications and treatments as prescribed.
  • Record and communicate changes in patient condition with physician.
  • Provide a safe environment and participate in group meetings.
  • Attend on-call duties and coordinate equipment/services.

Job Description:
  • Role: Registered Nurse - Home Health
  • Service Area: Devens, Shirley, Ayer, Pepperell, Townsend, Fitchburg, Leominster and Lunenburg areas.
  • Schedule: Monday-Friday (8:30-5:00pm)
  • Job Type: Full Time
Qualifications:
  • 2+ years' nursing experience, preferably in geriatrics is desirable.

Responsibilities:
  • Assesses and evaluates patient’s status by:
  • Writing and initiating plan of care.
  • Regularly re-evaluating patient and family/caregiver needs.
  • Participating in revising the plan of care as necessary.
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Provides and maintain a safe environment for the patient.
  • Attends interdisciplinary group meetings.
Additional Duties:
  • Participates in on-call duties.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Actively participates in quality assessment performance improvement teams and activities
