Job summary

Pathways Healthcare, a leading home health and hospice provider in Massachusetts, seeks a Registered Nurse to join our physician- and nurse practitioner-led team. The position is based in Concord with service coverage for the Lowell area, offering a flexible schedule and AI-assisted documentation to save clinicians time.

Qualifications

• Active RN license in the state where the position is based.
• Preferably 2+ years of nursing experience with geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust plans of care for each patient.
• Responsively address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of care planning.
