Job summary

Pathways Healthcare is seeking a Registered Nurse - Home Health to assess and manage patient care in Lawrence, MA, serving Methuen and surrounding areas. The role runs Monday–Friday, 8:30–5:00, with full-time hours.

Requires 2+ years nursing experience (geriatrics preferred), BSN, and valid driver's license. EMR proficiency, BLS/CPR, and regulatory knowledge are essential for delivering high-quality home health care.

Qualifications

• 2+ years nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient status; writes and initiates plan of care.
• Regularly re-evaluates patient and family/caregiver needs; revise care plan as necessary.
• Develops care plans based on nursing diagnosis and palliative actions; involves patient and family.
• Administers medications and treatments as prescribed by the physician.
• Completes and submits accurate clinical notes; records pain/symptom changes.
• Communicates with physician regarding patient needs; follows up on orders.
• Ensures a safe patient environment; attends interdisciplinary meetings.
• Participates in on-call duties; oversees ancillary staff.

Responsibilities

Patient Care

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities
