Pathways Healthcare is seeking a Registered Nurse - Home Health in Concord, MA to provide patient-centered in-home care. Service area includes Devens, Shirley, Ayer, Pepperell and surrounding towns.

Schedule is Monday–Friday, 8:30–5:00, full-time. Requires 2+ years nursing experience, nursing degree, EMR proficiency, and a valid driver’s license with reliable vehicle.

Qualifications

• 2+ years nursing experience, preferably geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsabilités

• Assess and evaluate patient status; write and initiate plan of care.
• Re-evaluate patient and family/caregiver needs; revise plans as necessary.
• Develop care plans with goals and nursing diagnoses; involve patient and family.
• Administer medications and treatments as prescribed.
• Record accurate clinical notes; track pain and symptom changes.
• Communicate with physicians; obtain orders as required.
• Maintain a safe patient environment and attend interdisciplinary meetings.
• Participate in on-call duties and ensure equipment and services are available.
• Supervise ancillary staff and support professional development.

• Salary: $72,000-92,000/year


• Main Office location: Concord, MA


• Service Area: Devens, Shirley, Ayer, Pepperell, Townsend, Fitchburg, Leominster and Lunenburg areas.


• Schedule: Monday-Friday (8:30-5:00pm)


• Job Type: Full Time

Qualifications:

• 2+ years’ nursing experience, preferably in geriatrics is desirable.

Responsibilities:

Patient Care:

• Assesses and evaluates patient’s status by:


• Writing and initiating plan of care.


• Regularly re-evaluating patient and family/caregiver needs.


• Participating in revising the plan of care as necessary.


• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.


• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.


• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.


• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.


• Provides and maintains a safe environment for the patient.


• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.


• Ensures that arrangements for equipment and other necessary items and services are available.


• Supervises ancillary personnel and delegates responsibilities when required.


• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.


• Fulfills the obligation of requested and/or accepted case assignments.


• Actively participates in quality assessment performance improvement teams and activities
