Job summary

Pathways Healthcare is seeking a Registered Nurse - Home Health to provide patient-centered care in the Kingston, PA area. The RN will initiate preventive and rehabilitative nursing procedures, administer medications, and develop plans of care in collaboration with physicians and interdisciplinary teams.

Ideal candidates hold an active nursing license in the state, have geriatrics experience, and maintain BLS CPR certification.

Qualifications

• Must hold an active nursing license in the state where based.
• Preferably 2+ years of nursing experience focusing on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust plans of care for each patient.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Oversee home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan.
