Job summary

Pathways Healthcare is hiring a Hospice Registered Nurse to join our physician- and nurse practitioner-led home health and hospice team. The role offers a full-time schedule, a competitive annual salary range of $78K–$95K, and serves Fall River and surrounding areas from our Westwood, MA office.

You will assess patients, develop and revise care plans, manage medications, and collaborate with the physician and interdisciplinary teams to deliver compassionate, patient-centered care in the home

Qualifications

  • 2+ years nursing experience; geriatrics preferred.
  • New nurses welcomed; training program provided.
  • Degree in Nursing from an accredited institution.
  • Proficient with electronic medical records (EMR).
  • Excellent bedside manner and communication skills.
  • Working knowledge of health care laws and regulations.
  • Maintain current BLS CPR certification.
  • Must possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

  • Assesses and evaluates patient status and writes initiaI plan of care.
  • Regularly re-evaluates patient and family/caregiver needs.
  • Participates in revising the plan of care as necessary.
  • Develops a care plan with goals based on nursing diagnoses and pallia­tive actions.
  • Initiates preventive and rehabilitative nursing procedures; administers medications as prescribed.
  • Completes and submits accurate clinical notes; tracks pain/symptom changes.
  • Communicates changes to the physician and follows orders as required.
  • Provides and maintains a safe patient environment.
  • Attends interdisciplinary group meetings.

Job Description:
  • Role: Hospice Registered Nurse
  • Salary Range: $78K-95K/Annual Salary
  • Main Office location: Westwood, MA
  • Service Area: Fall River and Surrounding Areas

  • Job Type: Full Time

  • Writing and initiating plan of care.

  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Provides and maintains a safe environment for the patient.
  • Attends interdisciplinary group meetings.
Additional Duties:
  • Participates in on-call duties.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Actively participates in quality assessment performance improvement teams and activities.
