Job summary

The Pennant Group’s Manito Home Health is seeking a full‑time Registered Nurse Case Manager to serve patients across Spokane and surrounding communities. This role offers autonomy, meaningful patient relationships, and the chance to deliver high‑quality home health care.

Responsibilities include comprehensive nursing assessments, in-home interventions, and coordinating care with an interdisciplinary team. Washington RN license required; up to date driving license and reliable transportation

Qualifications

• Current Washington RN license (or ability to obtain)
• One year of nursing experience preferred; home-health experience a plus
• Strong assessment, communication, and critical-thinking skills
• Ability to work independently in a field-based role; reliable transportation and valid driver’s license

• Complete comprehensive nursing assessments and develop individualized care plans
• Provide skilled nursing interventions in patient homes
• Manage a balanced caseload with strong organizational efficiency
• Coordinate care with interdisciplinary team members
• Educate patients and caregivers on disease processes, medications, and safety
• Maintain accurate, timely documentation
• Communicate effectively with physicians, families, and internal team members
• Support safe transitions from hospitals, SNFs, and outpatient settings

Wage: $45-55 per hour DOE plus mileage reimbursement
