Job summary

Columbia River Home Health in the Tri-Cities area seeks a compassionate Home Health Registered Nurse Case Manager (RN) to lead patient care at home. You will perform intake assessments, create care plans, and coordinate with physicians and families to support disease management and safety.

Qualifications

• Current RN license in Washington State.
• Minimum 2 years nursing experience.
• Home health experience preferred but not required.

• Strong communication, clinical judgment, and patient-focused care skills.

Responsibilities

What You’ll Do as an RN Case Manager
