Job summary

Manito Home Health, part of The Pennant Group, seeks a full-time Registered Nurse Case Manager to deliver in-home nursing care across Spokane and neighboring areas. You will assess patients, plan individualized care, coordinate with an interdisciplinary team, educate families, and ensure accurate documentation.

The role emphasizes autonomy, professional growth, and direct patient relationships in a home setting, with a daytime, Monday–Friday schedule.

Qualifications

• Complete nursing assessments and develop individualized care plans.
• Provide skilled nursing interventions in patient homes.
• Manage a balanced caseload with strong organizational efficiency.
• Coordinate care with interdisciplinary team members.
• Educate patients and caregivers on diseases, medications, and safety.
• Maintain accurate, timely documentation.
• Communicate effectively with physicians, families, and internal team members.
• Support safe transitions from hospitals, SNFs, and outpatient settings.

Responsibilities

• Full-time, Mon–Fri, daytime schedule.
• On-site/home-based duties within Spokane and surrounding communities.
