Elite Home Health & Hospice in Moscow, Idaho, is seeking a dedicated Registered Nurse (RN) Case Manager to coordinate and deliver high-quality home health and hospice services. • Current Idaho RN license in good standing.
• Graduate of an accredited nursing program.
• One year of nursing experience required.
• Home health or hospice experience preferred.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and manage a caseload.
• Current driver's license and reliable transportation.

Responsibilities

• Complete comprehensive patient assessments and develop individualized care plans.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Provide skilled nursing care according to physician orders and agency policies.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families about disease management, medications, and plan of care.
• Maintain accurate and timely clinical documentation.
• Participate in interdisciplinary team meetings and case conferences.
• Ensure compliance with federal, state, and agency regulations.
