Job summary

Hartford HealthCare Medical Group is seeking an RN to provide skilled nursing care in a multi-location CT network. Under provider direction, you will assess needs, plan and implement care, and support patients and families across clinics.

The role requires CT RN licensure, CPR certification, and experience with medical software. Travel to regional offices is required; the shift is Mon-Fri with a focus on patient-centered care.

Qualifications

• Current state of Connecticut RN license and CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing intervention promptly and professionally.
• Assists patients and family members with concern and confidentiality.
