Responsibilities include triaging calls, implementing nursing interventions promptly, and supporting patients and families with empathy while maintaining confidentiality. Some travel to regional offices is required.

Qualifications

• Active Connecticut RN license
• Current CPR certification
• Three years of medical practice experience preferred

Responsibilities

• Provides office-based nursing care in collaboration with provider
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care
• Demonstrates independent nursing actions based on assessment and problem identification
• Triages telephone calls and initiates appropriate nursing intervention
• Assists patients and family members with concerns, respecting confidentiality and privacy
• Travel to other regional offices as needed
