Job summary

Hartford HealthCare Medical Group is seeking an experienced registered nurse to provide office-based nursing care under provider direction across Connecticut locations. You will assess, plan, intervene and evaluate patient needs in collaboration with the care team.

Travel to regional offices as needed. Requirements include CT RN license, CPR certification, and 3 years' experience; experience with medical software is preferred.

Qualifications

• Current Connecticut RN license and CPR certification are required.
• Three years of experience in a medical practice preferred.
• Experience with medical software is preferred.

Responsibilities

• Provides office-based nursing care in collaboration with provider
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through nursing evaluation and intervention
• Demonstrates independent nursing actions based on assessment and problem identification
• Triages telephone calls and initiates appropriate nursing intervention promptly and professionally
• Assists patients and family members with concern and empathy; respects confidentiality and privacy
