Under the supervision of the Nurse Supervisor, you will perform assessments, develop care plans, administer medications, and collaborate with an interdisciplinary team to improve patient outcomes.

Qualifications

• ADN or BSN from an accredited program.
• Active RN license with no encumbrances.
• BLS/CPR certification current.
• Experience in long-term, post-acute, geriatric care preferred.
• Proficient English communication for documentation and patient safety.
• Health screenings including TST per state guidelines.

Responsibilities

• Perform comprehensive clinical assessments and develop individualized care plans.
• Administer medications and treatments and monitor patient responses.
• Lead and delegate to LPNs and CNAs within state practice acts.
• Coordinate care with physicians and interdisciplinary teams for safe discharge.
• Maintain real-time PCC documentation and ensure regulatory compliance.
• Mentor new nursing staff and support team growth.
