Ambercare Homecare is seeking a Registered Nurse to join our Deming, NM team. This full-time role runs Monday through Friday with rotating call weeks, offering a strong culture and collaborative team environment.

Qualifications

• Must be a graduate from an accredited RN program and licensed in NM.
• At least 1 year of clinical experience; home health preferred.
• CPR certification required.
• Must have reliable transportation and valid driver's license.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment, planning, implementation, and evaluation of the nursing process.
• Coordinate care with interdisciplinary team and attending physicians for optimal patient outcomes.
• Assess data from physical, psychological, social, and spiritual factors affecting patient and family needs.
• Initiate communication with the attending physician and other agencies to coordinate care.
• Maintain accurate patient records, progress notes, and timely care plans.
• Provide visits to assess patient needs and update the plan of care.
• Perform clinical nursing procedures as directed by the physician.
• Provide support to family and perform death pronouncement as licensure allows.

RN What Youll Do

• Provide visits as assigned by the Clinical Services Manager including the assessment determination of eligibility admissions processing planning implementation and evaluation phases of the nursing process
• Assignments will vary depending on licensure
• Obtain data and assess necessary information from patient physical psychological social and spiritual factors that may impact patient and familys needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patients needs and update the plan of care
• Perform therapeutic preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family as applicable to licensure
• Maintain accurate patient records charts progress notes reports and prepare care plans timely

Qualifications

• Registered Nurse
• Graduate from an accredited registered nursing program and licensed in the state
• At least one 1 year clinical practical nursing experience preferably in intensive care rehabilitation or medical surgical nursing preferred
• Home health experience preferred
• Possess and maintain valid CPR certification
• Must have reliable transportation current drivers license and appropriate automobile insurance
