Job summary

Ambercare Homecare in Albuquerque, NM is seeking a Registered Nurse for full-time consideration. Schedule: M-F 8–5 with a weekend admission rotation. You will provide nursing visits, assess patient needs, update care plans, and collaborate with the clinical services team to deliver high-quality home health care.

The role requires an RN license in the state and CPR certification, with preferred experience in home health and a strong commitment to patient-centered care.

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• At least one year clinical nursing experience preferred (ICU, rehab or med-surg).
• Home health experience preferred.
• Must have valid CPR certification.
• Must have reliable transportation, valid driver’s license and auto insurance.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment, eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data and assess information from patient physical, psychological, social and spiritual factors that may impact needs and coordinate intervention by the interdisciplinary team.
• Initiate communication with the attending physician and interdisciplinary team for coordinating optimal care.
• Provide visits to assess patient needs and update the plan of care.
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the physician.
• Perform clinical pronouncement of death and provide support to family as applicable.
• Maintain accurate patient records, charts, progress notes, and prepare care plans timely.

RN What You’ll Do:

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  • Home health experience preferred
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
