Job summary

Ambercare Homecare is hiring a Registered Nurse to join our growing team serving Silver City NM and surrounding areas. You will perform initial assessments, develop and implement plans of care, and provide direct patient care under the State Nurse Practice Act.

The role requires an NM RN license, CPR certification, and reliable transportation. Home health experience is preferred; 1 year of clinical nursing experience is desirable.

Qualifications

• Graduate from an accredited registered nursing program and licensed in NM.
• Minimum 1 year of clinical nursing experience, preferably ICU/rehab/med-surg; home health experience preferred.
• CPR certification required.
• Reliable transportation, valid driver’s license, and automobile insurance.

Responsibilities

• Complete initial patient and family assessment to determine home care needs.
• Provide comprehensive physical assessment and health history.
• Document observations in the clinical record and notify supervisor.
• Regularly re-evaluate nursing needs and revise the care plan as patient status changes.
• Administer medications and treatments as prescribed by the provider.
• Coordinate patient care and communicate with providers and family.

What Youll Do

• Completes an initial assessment of patient and family to determine home care needs
• Provides a complete physical assessment and history of current and previous illnesses
• Observe and report any signs or symptoms indicative of changes in condition of the patient and family situation
• Document such reports and observations in the clinical record and notify supervisor
• Regularly re evaluates patient nursing needs
• Initiates the plan of care and makes necessary revisions as patient status and needs change
• Uses health assessment data to determine nursing diagnosis
• Develops a care plan which establishes goals based on nursing diagnosis and incorporates therapeutic preventive and rehabilitative nursing actions
• Includes the patient and the family in the planning process
• Initiates appropriate preventive and rehabilitative nursing procedures
• Administers medications and treatments as prescribed by the provider
• Provides direct patient care as defined in the State Nurse Practice Act
• Counsels the patient and family in meeting nursing and related needs
• Provides health care instructions to the patient as appropriate per assessment and plan of care
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient
• Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload
• Prepares clinical notes and updates the primary provider when necessary and at least every 60 days
• Communicates with all relevant providers regarding the patients needs and reports any changes in the patients condition; obtainsreceives orders as required
• Communicates with community health related persons to coordinate the care plan
• Performs other duties as assigned

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state
• At least one 1 year clinical practical nursing experience preferably in intensive care rehabilitation or medical surgical nursing preferredHome health experience preferred
• Possess and maintain valid CPR certification
• Must have reliable transportation current drivers license and appropriate automobile insurance
