Job summary

Ambercare Homecare in Belen, NM is seeking a Registered Nurse to join our expanding team. This full-time role runs Monday through Friday, 8:00 am to 5:00 pm, with patient visits scheduled by the Clinical Services Manager and responsibilities spanning assessment, planning, implementation and evaluation of the nursing process.

You will coordinate with the interdisciplinary team, ensure accurate records, and provide compassionate, high-quality home health care.

Qualifications

• Graduate from an accredited RN program and licensed in NM.
• At least 1 year clinical experience, home health preferred.
• Maintain CPR certification.
• Reliable transportation, valid driver's license and insurance.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment, eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process.
• Collect data from patients and coordinate interventions with the interdisciplinary team.
• Communicate with attending physicians and other agencies to coordinate care.
• Update the plan of care based on patient needs and reassess.
• Perform nursing procedures, therapeutic and rehabilitative activities as directed.
• Declares death when applicable and provide family support.
• Maintain accurate patient records, charts, and care plans.

RN What You’ll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  • Home health experience preferred
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
