Job summary

Ambercare Homecare in Lordsburg, NM seeks a Registered Nurse to join our growing home health team. Full-time, Monday–Friday, 8am–5pm, with comprehensive benefits and a focus on patient-centered care.

The RN will perform assessments, plan and implement the nursing care, coordinate with the interdisciplinary team, and provide support to patients and families while ensuring quality documentation and compliance with licensure requirements.

Qualifications

• Must be a licensed RN in New Mexico.
• At least 1 year of clinical nursing experience; home health preferred.
• CPR certification required.

Responsibilities

• Provide visits as assigned by Clinical Services Manager including assessment, eligibility, admissions, planning, implementation and evaluation phases of the nursing process.
• Assess data from patient, family, and factors impacting needs, coordinating with interdisciplinary team.
• Communicate with physician and teams to coordinate optimal care.
• Provide visits to assess needs and update the plan of care.
• Perform nursing procedures as directed by physician.
• Handle clinical pronouncements of death and support family as applicable.
• Maintain accurate patient records and care plans timely.

Education

Graduate from an accredited registered nursing program and licensed in the state

RN What Youll Do

• Provide visits as assigned by the Clinical Services Manager including the assessment determination of eligibility admissions processing planning implementation and evaluation phases of the nursing process Assignments will vary depending on licensure
• Obtain data and assess necessary information from patient physical psychological social and spiritual factors that may impact patient and familys needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patients needs and update the plan of care
• Perform therapeutic preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family as applicable to licensure
• Maintain accurate patient records charts progress notes reports and prepare care plans timely

Qualifications

• Registered Nurse
• Graduate from an accredited registered nursing program and licensed in the state
• At least one 1 year clinical practical nursing experience preferably in intensive care rehabilitation or medical surgical nursing preferred
• Home health experience preferred
• Possess and maintain valid CPR certification
• Must have reliable transportation current drivers license and appropriate automobile insurance
