Job summary

Ambercare Homecare in Las Cruces, NM is seeking a Registered Nurse for a full-time on-site role. You will provide visits, assess patients, plan and implement the nursing process, and coordinate with the interdisciplinary team.

The position runs Monday–Friday, 8am–5pm, with favorable benefits and opportunities for growth. Prior home health experience and active RN license in New Mexico are preferred to join our compassionate care team.

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• One year clinical experience preferred in ICU, rehabilitation or medical-surgical nursing.
• Home health experience preferred.
• CPR certification required.
• Must have reliable transportation, valid driver’s license and automobile insurance.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment, eligibility determination, admissions processing and planning, implementation and evaluation of the nursing process.
• Gather data from patient and family across physical, psychological, social and spiritual factors and coordinate with interdisciplinary team.
• Initiate communication with attending physician and interdisciplinary team to coordinate optimal care.
• Provide visits to assess patient needs and update the plan of care.
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the physician.
• Perform clinical pronouncement of death and support the family as applicable to licensure.
• Maintain accurate patient records, charts, progress notes and care plans.

RN What You’ll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  • Home health experience preferred
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
