Job summary

Reliant Medical Group, part of Optum, seeks a Registered Nurse (RN) to join our Holden, MA Family Practice team. The role supports patient care across clinics with a 38.75-hour weekly schedule, Monday–Friday and rotating hours between 8:00 AM–5:00 PM.

You will provide direct patient care, coordinate with a multidisciplinary team, document in EMR, administer medications, and participate in care planning to improve outcomes. MA RN license required; BLS preferred; EPIC experience a plus.

Qualifications

• Graduate of an accredited School of Nursing.
• Licensed RN in Massachusetts at time of hire.
• Proficiency with Microsoft Word, Excel and Outlook.

Responsibilities

• Provides direct patient care directly to patients or through telephone management.
• Collaborates with multidisciplinary team to制定 and implement care plans.
• Documents all aspects of patient care and education in EMR.
• Administers medications and monitors responses per policies.
• Performs nursing interventions and participates in quality improvement.
