Job summary

Reliant Medical Group, part of Optum, seeks a Registered Nurse to deliver compassionate patient care in Holden, MA within a Family Practice team. In this role you’ll work in a clinically supportive environment with the resources of a national health system behind you.

The full-time position (38.75 hours weekly, Monday–Friday with a rotating schedule 8:00 AM–5:00 PM) offers a $2,500 sign-on bonus for external candidates, comprehensive benefits, and opportunities to grow.

Qualifications

• Graduate of an accredited School of Nursing.
• Currently licensed as a RN / LPN in the State of Massachusetts.
• Knowledge of software applications such as Microsoft Word, Excel and Outlook.

Responsibilities

• Provides effective patient care directly to patients or through telephone management.
• Delivers care based on patient's specific needs and priorities.
• Plans for care collaboratively with the patient and healthcare team.
• Implements interventions and evaluates the plan based on outcomes.
• Documents all aspects of patient care and education; reports to physician.
• Utilizes EMR tools to ensure primary and preventive services are accomplished.
