Job summary

Elara Caring is seeking a Registered Nurse to provide in-home health care, performing assessments, planning and coordinating care for patients in their homes across our branch area.

You will conduct initial and ongoing evaluations, develop and adjust care plans, and communicate with clinicians to ensure quality, compliant care. Travel is required and confidentiality under HIPAA is essential.

Qualifications

• Current, unrestricted RN license.
• 1+ year experience in a clinical care setting as a nurse.
• Able and willing to travel within branch/office coverage area.
• Capable of lifting and moving up to 50 pounds and equipment.
• Must have driver’s license or reliable transportation for patient home travel.
• Willing to travel 50% of the time.
• Associates or Bachelor’s in Nursing preferred.

Responsibilities

• Provide registered nurse home health care in patients’ home settings.
• Assess caseloads to identify physical, psychosocial, and environmental needs.
• Assume responsibility for caseloads: assess, plan, coordinate, implement, evaluate.
• Incorporate physical, psychological, social, and spiritual factors into care plans.
• Conduct initial evaluations and re-evaluate nursing needs at each visit.
• Make follow-up visits per patient, family, and facility needs.
• Communicate findings and changes to the Clinical Supervisor and team.
• Revise plans of care as issues arise and notify the team.

Job description

• Provide registered nurse home health care in patients’ preferred home settings
• Align all activities with Elara Caring’s vision and the leadership of the Home Health team
• Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
• Assess assigned caseloads of home health care patients and families to identify physical, psychosocial, and environmental needs
• Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating plans of treatment
• Assess patient needs and incorporate physical, psychological, social, and spiritual factors into plans of care
• Conduct initial evaluations and re-evaluate patients’ nursing needs during each visit
• Make follow-up visits according to patient, family, and facility needs
• Communicate significant findings, problems, and changes in health condition or environment to the Clinical Supervisor, physician, facility, and other care personnel
• Revise plans of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members

Requirements

• Current, unrestricted state RN licensee
• 1+ year experience in a clinical care setting as a nurse
• Able and willing to travel within branch/office coverage area
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Must be able and willing to travel 50%
• Associates Degree or Bachelor’s Degree in Nursing is preferred
• Previous Home Health or Hospice Experience is preferred
• Previous experience with HomeCare HomeBase is preferred
• Must complete Form I-9; Elara Caring participates in E-Verify after a job offer is accepted
