Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management.
• Complete scheduled visits, admissions, and clinical interventions.
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care.
• Educate patients, families, and caregivers on symptom management and end-of-life care.
