Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to coordinate end-of-life care for patients and families in the New Philadelphia area. You will develop individualized care plans, perform skilled nursing visits, and collaborate with physicians and the interdisciplinary team to ensure comfort and dignity.

Active RN license is required and hospice or home health experience is preferred.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete scheduled visits, admissions, and documentation in Homecare Homebase (HCHB).
• Respond to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and outcomes.
