Job summary

The Care Team Hospice is seeking a compassionate Hospice Registered Nurse Case Manager to coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort and quality of life. This role emphasizes patient- and family-centered care in a home-based setting.

Active RN license is required, hospice experience preferred, and familiarity with HCHB is welcomed.

Qualifications

• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete visits, admissions, and interventions with accurate documentation in HCHB.
• Respond to patient/family needs, including after-hours situations, to ensure patient-centered care.
• Collaborate with physicians and the interdisciplinary team to coordinate care and support outcomes.
• Educate patients and families on symptom management and end-of-life care.
