Qualifikationen

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Aufgaben

• Provide skilled nursing care, assessments, and symptom management per hospice standards and patient plans.
• Complete scheduled visits, admissions, and interventions with accurate documentation in HCHB.
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support positive outcomes.
• Educate patients, families, and caregivers on symptom management and end-of-life care with compassion.
