Zusammenfassung

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our Warren, OH team. You will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients and families.

Active RN license and hospice experience are preferred, with familiarity in Homecare Homebase.

Qualifikationen

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

• Complete scheduled visits, admissions, and clinical interventions with timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care and support patient outcomes.
• Educate patients and families on symptom management and end-of-life care.
