Springhill Hospice in Mobile, AL is seeking a dedicated RN Case Manager to provide patient-centered hospice care. You will assess needs, develop care plans, administer nursing care, and support families while collaborating with an interdisciplinary team.

Three or more years of hospice experience are preferred. A valid nursing license and reliable vehicle are required to ensure timely patient visits and on-call coverage.

Competenze

• Graduate of an accredited nursing school with state licensure.
• Minimum 1 year nursing experience; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a reliable vehicle.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
