Job summary

Hospice of North Alabama is seeking a dedicated RN Case Manager to provide patient-centered hospice nursing care in Florence, AL, supporting patients and families with comprehensive care plans and ongoing assessments.

The role emphasizes collaboration with an interdisciplinary team, adherence to hospice philosophy, and timely documentation, with regular on-call shifts and a Monday–Friday schedule.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured automobile.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
