Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Registered Nurse Case Manager for Hospice in Dare and Currituck counties. The role focuses on 1:1 patient care, creating individualized hospice plans, and collaborating with a multidisciplinary team in a home-based setting.

The position offers weekday hours, on-call rotation, and paths for clinical excellence and career growth within a expanding company. Requirements include RN licensure in NC and CPR certification.

Qualifications

• Graduate of an accredited nursing school with current licensure in state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile and valid insurance.
• Possess and maintain CPR certification.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
