Job summary

Adoration Health is seeking a Hospice RN Case Manager to join our Cleveland, MS team. This role focuses on compassionate, patient-centered hospice care with a strong emphasis on collaboration and documentation excellence.

Responsibilities include assessing patient needs, developing care plans, administering treatments, and educating families. A valid RN license in Mississippi, 1 year of nursing experience, and a driver’s license are required, with hospice experience preferred.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the state of operation.
• One year nursing experience required, 3+ years preferred.
• Hospice care experience preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have valid driver’s license and reliable vehicle.

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:
