Job summary

Adoration Health is seeking a dedicated Hospice RN Case Manager to provide patient-centered care in the Nags Head, NC area. You will assess patient needs, develop individualized hospice care plans, administer treatments, manage pain and symptoms, and support families while collaborating with a cross-functional team.

The role requires RN licensure, CPR certification, and at least 1 year of nursing experience (3+ years preferred), with a valid driver’s license.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• Minimum 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must be a licensed driver with an automobile that is insured in accordance with state requirements.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
