Adoration Health seeks a dedicated Hospice RN Case Manager in Nags Head, NC to provide compassionate, 1:1 patient care across the hospice care continuum. You will develop and follow care plans aligned with patient goals, administer medications and therapies, and support families with education and resources.

The role emphasizes independent practice with team support, ensuring clinical and documentation excellence while contributing to a growing home-based care mission.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:
