Job summary

Haven Hospice in Gainesville, FL is seeking an experienced RN Case Manager to join our team. You will provide 1:1 patient care, develop and implement hospice care plans, and collaborate with a compassionate interdisciplinary team to ensure the patient’s goals are met.

Schedule is FT, Monday through Friday, 8am-5pm with no weekends or holidays. You will assess needs, manage symptoms, educate families, and participate in on-call rotations as required by the local branch.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with an automobile that is insured in accordance with state requirements.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate on-call rotation as required by local branch.
