Job summary

Haven Hospice in Gainesville, FL is seeking a passionate Hospice Admissions RN to join our team. You will visit patients in homes, hospitals, and facilities, and explain hospice services while gathering medical history and coordinating care with families.

As part of the admissions team, you will coordinate service starts, collaborate with an interdisciplinary team, and maintain accurate documentation. Benefits include insurance options, retirement, PTO, and more.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year hospice nursing experience required, 3+ years preferred or in similar settings such as Long-Term Care, ICU, or ED will be considered as substitute.

Responsibilities

• Visit patients in homes, hospitals, nursing facilities, etc. referred for hospice care.
• Explain hospice services, discuss goals with patient and families, obtain consents and documents.
• Obtain medical history, current medications, and symptoms; perform comprehensive assessments.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team and maintain timely documentation.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required , 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
