Job summary

Haven Hospice in Lake City, FL is seeking a compassionate Registered Nurse to join our hospice team as an RN Case Manager. This full-time role provides patient-centered care in home settings, emphasizes 1:1 support, and requires driving across designated service areas.

You will assess needs, develop care plans, deliver medications and treatments, and collaborate with an interdisciplinary team. A valid Florida RN license, and 1 year nursing experience (3+ preferred) are required; hospice

Qualifications

• Graduate of a nursing program with current Florida RN license.
• One year nursing experience required; three+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care and clinical documentation.
• Valid driver's license with reliable insurability.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create and adjust hospice care plans aligned with patient goals.
• Direct nursing care: administer medications, treatments, interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
