Job summary

Haven Hospice in Gainesville, FL, is seeking a part-time Hospice Admissions RN for PRN 10-hour shifts, every other weekend (Sat/Sun). You will travel to patients' homes and care settings to coordinate hospice admissions, discuss goals, obtain consents, and assess eligibility.

Responsibilities include comprehensive history taking, medication review, and documenting care with a supportive interdisciplinary team. A valid RN license and driving need are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care or Emergency room will be considered in place of hospice nursing experience.
• Must be a licensed driver with an automobile that is insured and in good working order.

• Explain hospice services, discuss goals and care with the patient and families, answer questions, and obtain necessary consents and legal documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
