Job summary

Springhill Hospice in Daphne, AL is seeking a compassionate Hospice RN Case Manager to provide patient-centered care. You will assess needs, develop care plans, and deliver direct nursing care including medications and interventions within a supportive team environment.

The role emphasizes pain management, family education, and collaboration with an interdisciplinary team, with on-call rotation as required by the local branch. Licensure and CPR are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with insured vehicle
• Possess and maintain CPR certification where required

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with patient wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers

As a RN Case Manager You will:

Qualifications
