Job summary

Hospice of North Alabama in Florence, AL is seeking an RN Case Manager to assess/monitor patients' physical, emotional, and spiritual needs, develop care plans, administer treatments, and collaborate with the interdisciplinary team.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured... and in good working order

Responsibilities

• Participate in on-call rotation as required by local branch

Schedule: Monday thru Friday, 8am-5pm
