Job summary

Adoration Health in Prattville, AL is seeking a dedicated Hospice RN Case Manager to join our team. You will provide patient-centered hospice care, develop care plans, administer treatments, and collaborate with interdisciplinary staff to support patients and families at a difficult time.

We value clinical excellence, thorough documentation, and compassionate care. A valid nursing license and driver with insured vehicle are required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured per state/organization requirements and in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will
