Job summary

Springhill Hospice, part of BrightSpring Health Services, is seeking a passionate Registered Nurse Case Manager for Hospice in Mobile, AL. The RN will deliver patient-centered end-of-life care, coordinate care plans, and collaborate with an interdisciplinary team to ensure comfort and dignity for patients and families.

The role offers a Monday–Friday schedule with on-call rotation as needed, and a focus on compassionate, evidence-based hospice care in the Mobile community.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
