Springhill Hospice in Daphne, AL is seeking a dedicated Hospice RN Case Manager to deliver compassionate, patient-centered care in home settings within Baldwin County. You will assess needs, develop care plans, administer medications, and educate families, collaborating with an interdisciplinary team.

Qualified candidates hold an RN license, with 1 year of nursing experience (3+ years preferred) and CPR certification. We offer a supportive environment, flexible scheduling, and excellent benefits.

Qualifications

• Current nursing licensure in state of operation.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• CPR certification maintained.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient goals.
• Direct nursing care: medications, treatments, interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate documentation.
• Participate in on-call rotation as required.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
