Job description

The Registered Nurse Case Manager (Gastroenterology) provides care, counseling of persons in the promotion and maintenance of health, prevention of illness, and injury based upon the nursing process within Medicine Service - Gastroenterology Section.

Qualifications

Basic Requirements:

• English Language Proficiency
• In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless proficient in basic written and spoken English.
• Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE at the time of completion.
• OR individuals attending a master's level bridge program in nursing with coursework equivalent to a bachelor's degree in Nursing may have the opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program.
• Upon achievement of a State license, the individual may be appointed on a temporary basis and later converted to a permanent appointment upon graduation.
• For foreign graduates, possession of a current, full, active, and unrestricted registration meets the graduation requirement from an approved school.
• Credit for foreign nursing education higher than an associate degree requires a formal degree equivalency validation.
• Current unrestricted registration as a graduate professional nurse in a U.S. State, Territory, or the District of Columbia.
• Graduate Nurse Technician (GNT) Exception: Candidates meeting other education requirements but lacking licensure may be appointed temporarily while pursuing licensure.
• Preferred experience includes 2-3 years in Ambulatory/Acute Care in Gastroenterology/Endoscopy or related fields.

Physical Requirements: Heavy lifting, moderate lifting, light lifting, carrying, pushing, reaching, use of fingers, walking, standing, bending, coordination, vision, hearing, stress management, and annual health requirements.

Duties

• Provides direct care to Veterans in ambulatory Gastroenterology clinics.
• Coordinates care for surgical services and performs comprehensive assessments and evaluations.
• Utilizes services such as case finding, health teaching, and counseling.
• Executes medical regimens, administers medications and treatments as prescribed.
• Collaborates with health team members to develop and implement patient care plans.
• Demonstrates knowledge of Gastroenterology & Endoscopic procedures and management.
• Provides patient-centered care, promotes positive outcomes, and utilizes resources effectively.
• Performs interdisciplinary practice to promote health, manage illnesses, and support surgical patients.
• Provides ongoing evaluation of care management and patient satisfaction.
• Ensures timely referrals to non-VA care when necessary.
• Utilizes advanced practice skills within scope of practice.
• Performs other duties as listed in the functional statement.
