Job summary

HCA Florida Northwest Hospital is hiring a Registered Nurse Case Manager responsible for supporting patients and families throughout their care journey. In this role, you'll coordinate services, develop care plans, and work closely with healthcare professionals to ensure quality patient outcomes.

Job Summary and Qualifications

As a Registered Nurse Case Manager, you will support patients and families through every step of their care journey, coordinate services, develop care plans, and collaborate with physicians, nurses, and department leaders to ensure safe transitions and consistent quality across the continuum of care.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
• Develop and implement individualized treatment plans reflecting the patient’s strengths, needs, and personal recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team.
• Collaborate closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient‑centered care.
• Promote HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction.

Qualifications

• Registered Nurse with current state licensure; BSN preferred.
• Three years of clinical, hospital nursing experience required, with preference for those having 2 years of case‑management or 2 years of critical care experience.
• Certification in case management or utilization review preferred.
• Experience with InterQual preferred.
