Job summary

HCA Florida Northwest Hospital in Florida, NY is seeking a Case Manager to support patients and families through the care journey, coordinating services and developing individualized care plans reflecting each patient’s needs.

You will partner with physicians, nurses, and department leaders to ensure safe transitions, clear communication, and consistent quality across the continuum of care, while leveraging RN licensure and preferred BSN.

Qualifications

• RN with current state licensure; BSN preferred.
• Three years' hospital nursing experience with preference for case management experience.
• 2 years of case management or 2 years of critical care experience.
• Certification in case management or utilization review preferred.
• InterQual experience preferred.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths, needs and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare's values of compassion, respect, and excellence in every interaction.

Job Summary And Qualifications

What qualifications you will need:

• RN with current state licensure; BSN preferred
• Three years' clinical, hospital nursing experience required with preference given to those with 2 years of case management experience
