Job summary

Qualifications

• RN with current state licensure; BSN preferred.
• Three years of acute-care nursing experience (Med-Surg, Tele, PACU, ICU, CVICU, etc.).
• Certification in case management or utilization review preferred.
• InterQual experience preferred.

Responsibilities

• Provide program orientation to patients, families, caregivers, covering role, stay criteria, Medicare/insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities.
• Act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, status through Family Conferences and other interactions.
• Ensure the implementation of the patient's treatment plan that supports strengths, abilities, needs, and preferences; facilitate the involvement of the patient throughout the rehabilitation process.
• Document findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments; communicate social, financial, or discharge needs and preferences of the patient/family/caregiver.
• Promote consistent, positive patient interactions that advance the agenda of unparalleled patient service.

Job Summary

The Case Manager (CM) ensures high‑quality, patient‑centered care by managing rehabilitative care to ensure optimum outcomes. Qualifications

• Three years of clinical acute‑care nursing experience (Med‑Surg, Tele, PACU, ICU, CVICU, etc.) with preference given to those with 2 years of case management or critical‑care experience.
• Certification in case management or utilization review preferred.
• InterQual experience preferred.
