Spectrum Healthcare Resources seeks a Registered Nurse - Case Manager to support Reid Medical Clinic at Lackland AFB in San Antonio, TX. The role focuses on coordinating patient care, referrals, and care continuity for military beneficiaries.
You will collaborate with clinicians and community resources to educate patients and families about treatment plans and wellness, ensuring readiness and health of those who serve. EOE/Disabled/Veterans.
Competenze
- U.S. Citizenship required.
- Associate Degree in Nursing or higher from an accredited program.
- Current, unrestricted RN license meeting Texas requirements.
- 3 years of RN Case Management experience within the last 4 years.
- Current BLS certification (AHA or American Red Cross).
- Case Management certification such as CCM, CDMS, CRRN, COHN, RN-NCM, or another PWS-approved certification
