Job summary

Spectrum Healthcare Resources is seeking an experienced Registered Nurse – Case Manager to support the Trainee Health Squadron at Reid Medical Clinic in San Antonio, TX. The role focuses on coordinating patient care, referrals, and continuity of services for a military healthcare setting.

Ideal candidates hold an ADN or higher, active Texas RN license, and CCM/CDMS/CRRN certifications. The position offers Monday–Friday hours with comprehensive benefits and paid time off.

Qualifications

• U.S. Citizenship required.
• Associate Degree in Nursing or higher.
• Current, unrestricted RN license meeting Texas requirements.
• 3 years of RN Case Management experience within the last 4 years.
• Current BLS certification (AHA or ARC; hands-on).
• Case Management certification such as CCM, CDMS, CRRN, COHN, RN-NCM, or other approved certs.

Responsibilities

• Coordinate individualized patient care and treatment plans.
• Manage referrals, follow-up care, and continuity of services.
• Collaborate with healthcare providers and community resources.
• Educate patients and families on treatment and wellness.
